This section contains Legal Releases for Oral History interviews, taped Hall of Fame Induction Ceremonies, and for speakers at Park Forest Historical Society Programs.  Also included is a Publicity Privacy Release which is to be used before using photographs of individuals in publicity of any kind, inlcuding the website.

Forms found here are the Park Forest Hall of Fame nomination form which must accompany any nomination, and the back of the form which lists all current members of the Hall of Fame. Administrative forms for the Hall of Fame Committee are found in this section, but have restricted access.

Loan Forms and collection maintenance forms will be found in this section.  This area is under construction.  Forms will be added.

Forms for working with Volunteers are found in this section.  An emergency form and an application must be filled out by each volunteer.  We record the number of  hours contributed by each volunteer as In-kind donations.  The In-kind donation form is in this section.

Park Forest Local History Collection and Archive

You will find here finding aids, collection lists, and any forms for use of the collection.
The collection does not have many formal finding aids in 2009, but we will post them as they are written in the future. We will post those we have access to in 2009.

Park Forest Photograph File Forms

These forms, use fee schedules and image price lists are to be used when someone wants to use an image in a publication of any kind, have a print made, have images copied to media or sent electronically, or wants to use an image in an exhibit. The agreement must always be signed by both parties.
